Surgery
We work with orthopaedic, vascular, plastic, cardiac, general, and other surgeons, and they find value in working with 2×2 Health as an extension of their patient care and support. 2×2 staff coordinate pre- and post-surgical patient care including related appointments, surgery preparation, medication schedules, wound care and regular physical therapy appointments as necessary. Research has demonstrated that this type of collaboration leads to better outcomes for both patients and their physicians.
Primary Care and Oncology
Whether it’s a health crisis, an overwhelming diagnosis, or ongoing treatment such as chemotherapy, we are with your patients every step of the way. We partner with primary care physicians, oncologists, and clinicians, providing the connection between your medical team, patients and their families.
Healthy Living
Good health starts with wellness and prevention. Our registered dietitians and nurses work with your patients to manage preventative and wellness care by seamlessly implementing the healthcare plan you’ve mapped out. Follow-up appointments are much more productive, and because our clients are part of developing their own plan, they have a stronger level of engagement and compliance.
Working with 2x2 Health has increased the speed by which we can identify any problems and quickly devise a plan to address issues with our pediatric patients. When working with patients and their families, I’ve learned over the years that many family members don’t see themselves as caregivers. We all give care at some point, that’s understood. But there’s certainly a difference between what we identify as a care team, and who patients see as part of their care team.
The hospital discharge process can be a stressful time for patients and their caregivers. I often relate it to getting ready step off a ledge: you’re not sure how high that first drop is, so it’s difficult to understand how things will go once you step forward.
