The two words “Care” and “Coordination” are positive and upbeat words—but what do they mean when we use them together? How do you know if you or your loved one is receiving healthcare in a coordinated way? Most of us are familiar with needing some type of medical attention throughout our lives. Depending on the severity of a healthcare concern, further coordination and communication could be necessary. When we have a minor healthcare issue, there are often just a few steps to take to get on the path to a full recovery. However, individuals may have a more complex healthcare challenge due to the type of illness, age, proximity and availability of their support system, and changes in environment. During times like these, specific attention to coordinating care and information is even more critical.
According to the Agency for Healthcare Research and Quality, “Care Coordination involves deliberately organizing patient care activities and sharing information among all of the participants concerned with a patient’s care to achieve safer and more effective care.” Research has shown that Care Coordination has many benefits to patients including but not limited to: a significant increase in patient survival, improved quality of care, improved clinical outcomes, and increased patient confidence in self-managing care. Care Coordination is one of the most influential activities a patient can engage in to keep from being readmitted into the hospital.
